I did this before but can't remember the particulars.

1. Name under which application was filed or Corp/Partnership/Assoc etc.
2. Name of person Certifying to Citizenship.

I left #1 blank and inserted the name of the Trust in the Corp space.
In #2 I put my name.

Anyone done one this week:D

I've seen too many on the net that have stated that they had to amend their
application to include it. I agree that it hardly could be considered necessary
when the entity can't be a citizen. It appears to be sporadic in the Dept. some
want it, some don't, if the ruling is changed it WILL be required. So I'm going
to send it anyway, I did it for the Form 4 stuff. Should have done this years
ago. The last thing you want to have happen is the app gets delayed with a
potential year long wait.

12 stamps, only sent it with the first 4...but you're right, painless to send it. My examiner (Dudash) doesn't require it.

Sarah Jones (whoever she is) did both of mine. She did mostly Texas, big
place with lots of toy sales. They moved her somewhere else now it's backed up big time.
